Vision/Hearing/Speech Screening

 

The Special Senses and Communications Disorders Act, Texas Health and Safety Code,

Chapter 36, requires that all children who are four and five years old as of Sept. 1,  2008,

and who are enrolled in a licensed child care facility have on file medical screening for

vision and hearing.  Screening is optional for children in our three year old classes.

 

This screening may be accomplished through the child’s personal physician

(your child’s physician may have already done this at their four year old check- up). 

We also have the option of on-site screening by Rothschild Speech-Language.

 

Attached is information regarding the screening by Rothschild Speech-Language.  The screening will take place at the ELC on Friday, November 14.   If you elect to not use this service, and if this information is not recorded on your four or five year old child’s Admission Health Form, we ask that you please give us a copy of Vision & Hearing Screening from your child’s physician by Friday, December 5 in order for us to be in compliance with licensing regulations. Reports from the doctor may be faxed to the church office at 972-234-2625 to the attention of the ELC.

 

If you have a three-year-old child who you would like to have screened and your child attends on Tuesday and Thursday, please arrange with Kristen to have your child screened on Friday, November 14.

 

If you choose to use Rothschild Speech-Language Services, please see Kristen in the office for the forms.  The forms and  payment are due by Friday, November14.

Extended Day Openings

 

How would you like two extra hours on Tuesday or Wednesday to have lunch with a friend, make a doctor appointment, shop or just have some quiet time? We still have a few openings in our classes for extended day on Tuesday and Wednesday. If you are interested in one of these extended days for your child, please talk to Kristen in the office. Extended

day adds $35 a month to your regular tuition.

In classes with openings, we also offer

“drop-in” extended day for $15 a day.

We appreciate 24 hour notice when possible.
